It's a huge topic---software evolution, requirements management, revision control &c. Every project needs its specific detailed solution to managing change and enforcing DB & app version integrity. There's no formula, but there's a lot of stuff written on the subject.
In your case, the app is going to have to administer DB upgrades and behave accordingly.
