With the scant detail you provide, it's hard to be specific.
Sounds like it could be your script checks for duplicate username. The test is faulty, and sees that any user is a duplicate, but doesn't tell you what went wrong.
Sounds like the php needs a LOT of work on error checking.
But then, maybe that isn't the problem at all!
