1,2) depends what your requirements are. sometimes people use separate fields for region, international and local parts of the phonenumber. I think I'd try using integers, but I'm afraid I do not have much experience with this one. You should definitely check this one out:

This will answer quite a few questions.
3) I'd use a decimal (storing just the numbers, NOT the percent sign).
4) Date is good if you dont need time. It saves storage and can simplify your queries.
