Realmente sua querie deverá ser otimizada. LEFT JOIN usando OR, é complicado amigo... vai demorar...
Informe o metadados da tabela pra gente melhorar essa consulta.
mysql> SHOW CREATE TABLE tabela;
é, o my.cnf pode ser melhorado sim...
