I see Dmitry has responded with the MySQL Sync tool which should do what you want if not in the way you want it. Right now, that tool (although I am not familiar with it) may be your only option without having to make modifications to your program.
If you are OK with mods, you might want to try recompiling your DOS application under Visual FoxPro and placing your tables in a DBC. The app SHOULD (note, that I said SHOULD, not WILL) run fine with those simple mods (you would obviously have to test it). Assuming it does, you could write some triggers to keep you data up to date in real time.
If real time is important enough to you, you may want to try option 2. Otherwise, try the Sync app.
