Fine, that is a good start, all you need to work on the code that will link your table to a web form. Perhaps purchasing a php/mysql is a good idea. Alternatively check out the 'articles' section (see previous post) of the site that point to articles such as this -

I do not not have any prior knowledge of 'cpanel' so am unable to say if this is an appropriate tool to use. Intitialy when first setting out with PHP I used a WYSIWYG program but moved to creating the code myself via a standard text editor.
