Connection Failed [HY000][MySQL][ODBC 5.1 Driver] Can't connect to MySQL

Connection Failed [HY000] [MySQ:]{ODBC 5.1 Driver] Host (host name here) is blocked because of many connection errors; unblock with “mysqladmin flush-hosts”

We keep flushing out the Host on the database, but the problem comes back again.

What is causing the hosts to fill up? What is causing the error?
