Duplicate User Names even though there is none

I have adapted and changed a PHP script and now when I enter information for a registered user it won't register them and it says there is a duplicate user name even though there is not. I am new to PHP and the MySQL database and I wonder if it would be possible if I accidently loaded the database through the FTP 2 times would this happen. Can you load the same database 2 times through FTP? If you go to the site I am working on it is http://www.ministryclub.com and click on either jobseeker registration or employer registration. If you fill in the information it doesn't work. Any ideas?
