Same issue here...
I'm running 4.1.12 on OS X. I have had multiple builds of 4.x on multiple versions of OS X. Ever since I switched to 4.x from 3.x I get this all the time. I have resorted to running a cron job every hour to run a myisamchk and 'REPAIR TABLE...' on my tables.
I have installed 4.x on multiple systems (some single drives, some RAID, OS X 10.2, 10.3 and 10.4) all end up with the same table corruption. The worst is when I run myisamchk I loose data. It seem to be when I create a new row in a table. Subsequent queries fail. The REPAIR TABLES... seems to fix it but it will randomly crop back up.
