I mean that instead of using an auto-increment on the integer primary key, clients manually specify the value of the integer primary key for inserts and those values are not monotonically increasing. E.g.:

Client 1 inserts record with PK=1
Client 1 inserts record with PK=2
Client 1 inserts record with PK=3
Client 2 inserts record with PK=10
Client 2 inserts record with PK=11
Client 1 inserts record with PK=4

Except this happens with lots of clients on several high-volume tables.
