Use Add/Remove App.
Go to the "Start Menu"
Click on "Control Panel"
Find the "Add Or Remove Programs" icon
Find and select "mysql-proxy"
Clicking on "Remove".

This method is the most common one. However, it sometimes does not work for all the program as it can't fully uninstall your mysql-proxy. So I suggest you use an Mighty Uninstaller, which can fully and completely remove mysql-proxy.
