OK, I see what is going on here. MySQL was unable to obtain enough RAM, and that was why it was resorting to repair by keycache. I dropped the index caches, set the myisam_sort_buffer_size to 10G, and it used filesort.

It would be really nice if errors were logged when repair by keycache was used, listing the reasons.
