Oracle SQL query not working in MySQL

Can anyone tell me why this will not work in MySQL. It works fine in Oracle.

SELECT a.fldMID AS "Id",
COALESCE(a.fldVALUE, 'none') AS "Disposition",
COALESCE(b.fldVALUE, 'none') AS "PTC Call Number",
COALESCE(c.fldVALUE, 'none') AS "Deployment",
COALESCE(d.fldVALUE, 'none') AS "Business Unit",
a.fldDATE as "Date"
FROM tbl_form_values
WHERE fldFID = 1
FROM tbl_form_values
WHERE fldFID = 2
FROM tbl_form_values
WHERE fldFID = 3
FROM tbl_form_values
WHERE fldFID = 4
WHERE a.fldMID = b.fldMID
AND a.fldMID = c.fldMID
AND a.fldMID = d.fldMID
AND a.fldVALUE = 'General'
AND b.fldVALUE is null
AND d.fldVALUE = 'NHD'
