Index creation involves sorting. Maybe this is the bottleneck in your case. Check
http://dev.mysql.com/doc/mysql/en/myisam-start.html for myisam_sort_buffer_size, myisam_max_extra_sort_file_size, myisam_max_sort_file_size .
