Isn't the index arranged something like this:
ColA ColB
1 1
1 2
1 3
1 4
2 1
2 2
2 3
2 4
If you're looking for all rows with colA > 1 and colB < 3, you go to the "2" section of colA in the index and then take all the rows in that section above "3" in the colB index. No table scanning necessary.
Or am I wrong?
