I just figured it out. This is a Windows 7 Enterprise installation, but may apply to other versions too.

The setting that causes the floating button to render incorrectly can be toggled by doing the following:

1) right+click "My Computer"
2) choose "Properties" from the context menu
3) click "Advanced System Settings"
4) choose the "Advanced" tab
5) choose Performance --> Settings
6) put the Visual Effects into Custom mode
7) toggle the option (at the bottom of my list) called "Use Visual Styles on Windows and Buttons"
8) go check the MySQL for Excel floating button

It's disappointing that this option must be enable for the button to render correctly as I like my desktop to be very minimalist, but this is the fix.
