Obvious question first, just in case. The server is set to use port 3306. Is that the port the external users are using and is the firewall set to let it through (must be, it seems...:)).

Next I wonder about thread_cache and how many connections you're getting. Might try a higher value for it. But I have a hard time seeing this explaining seconds worth of delay.

How fast is select version() from inside and outside? If it's slow that would point away from your query or the cache as the issue.
