Hello,
The problem arise when you upgrade an old MySQL server to a newer version.
From time to time, MySQL introduce new columns/tables and you have to run
the script "scripts/mysql_fix_privilege_tables" (located in the MySQL folder)
or you can run "share/mysql_fix_privilege_tables.sql"
