Initial results look promising on this index change. I tried it on the 20 million rows and got sub 0.1s performance. I will rebuild the indexes on the 200 million rows and do some more complete testing. Then I will post my findings. As you noted the filesort and temporary went away. (I think I had a different view of how the optimizer was solving the query, but when you brought up this change I think it final has dawned on me what the optimizer is trying to do.)
