It's often more efficient to store blobs directly on disk, and store paths to them in the db. Either way, you have to follows the regulations of the country where the system will run. US/HIPAA regs are by no means exemplary. Regs in Europe & Canada tend to be significantly more protective of privacy. Wherever you are, you are likely looking at a mountain of regulations to follow.
