This smells like a recent bug... think it may be fixed in the 5.1.7 release (going off memory here :)
Otherwise, I'm assuming your data directory was left around from the old install, but with running the fix priviledge table scripts?
I know the mysql_install_db script does the right thing in creating the tables...
