We just started getting precisely the same error. Have you resolved this issue?
We, are running Windows 2003, MySQL 4.1.7nt and our precise event, as logged was :
Faulting application mysqld-nt.exe, version 0.0.0.0, faulting module mysqld-nt.exe, version 0.0.0.0, fault address 0x001e6e9c.
Is there a way to further debug this so I can find what queries or actions might be causing this?
