I think that 3.0.16 might have something to do with it.

The DB is in the US (I'm in Australia). It was upgraded the other day.

Yesterday I upgraded to 3.0.16 (on the advice of a collegue - he said 3.0.15 wouldn't work with 4.1) and started to notice the slowness.
