I would also recommend Full Convert from Spectral Core. It will convert FoxPro DBF to MySQL very easily (or dBase DBF, of course).

There is also an online tutorial showing how to convert DBF to MySQL:

You can customize basically anything, including data type conversion rules, naming, default values, etc.

There are also many advanced features you may not need, like scheduler, external SQL script execution, etc. Full Convert supports many other databases, in case you need to convert some other databases, too.
